The fees that are charged in this office are comparable to those charged by other competent specialists in our fields.
The following is a list of insurance carriers we are contracted with, however, contractual agreements are subject to change and we recommend that you contact your insurance carrier prior to your appointment to verify that we are a contracted provider.

We ask that you call your insurance prior to our visit. Please verify that your policy is active and understand your insurance policy, referral requirements, limitations, deductibles, and your financial responsibilities.
Please bring your insurance card.
Copayment are collected at the time of your visit Statements are generally sent 7 to 30 days after your visit depending on your insurance coverage. This varies significantly for certain types of accounts, so please do not be concerned if yours takes longer. Payment is required in full upon receipt of your statement.
Uninsured patients are required to pay at the time of service.
All new cosmetic procedures require a cosmetic consult.. Advance notice of cancellation is required. If you fail to show up for your cosmetic procedure or do not provide 24 hour notice of a cancellation, partial or full cost of the procedure will be assessed. Some cosmetic services require appropriate scheduling and preparation; these services require a deposit in advance. Due to product costs and product preparation, Sculptra treatments require a $500 per vial deposit to schedule an appointment. Deposits are non-refundable.

Skin Care and Retail Products
We accept returns on retail items within 7 days of product purchase for account credit only if there is dissatisfaction with the product after minimal use or a product reaction. Returns are applicable for account credit only. We cannot accept returns on items requiring a prescription including Tri-Luma, and Latisse.
We also recommend CareCredit Patient Financing, a special program for cosmetic services. With CareCredit you can finance your cosmetic procedures without upfront costs, annual fees, or pre-payment penalties. http://www.carecredit.com/
Medspa Cancellation Policy
Treatments are selected and reserved specifically for your needs, based upon availability. In order to best accommodate all guests, we require 24-hour advanced notification of changes and cancellations. Cancellations within the 24-hour time period are subject to 50% cancellation penalty. No notification of a cancellation will result in a full charge for your treatments.
Gift cards are available for purchase and are applicable towards services only and not retail products. They are valid for one year from the date of purchase
