Policies for requesting an appointment

We encourage our patients to go through all of our policies before requesting an appointment. If you proceed and request an appointment you are acknowledging and accepting our policies.

This is only a request, your appointment will be confirmed by or by phone call
If you are scheduling for the same day you should also call in to confirm the appointment has been accepted.
If you need to re-schedule your appointment, please contact the practice directly instead of booking a new appointment.
Online scheduling is only for routine medical visits new and stablished, they are not for surgical or cosmetics procedures.
There is a cancelation fee $50 for appointments not cancelled within 24 hours.
Please include your insurance policy number when requesting an appointment.
